Youth Waiver - Every player must submit this form (must have original form, no scanned copies, no faxed copies)

Proof of Age - Copy of Player Cards (USYS or US Club Soccer), copy of Birth Certificate or Passport for every player.
Submitting Paperwork for Session 1 and/or 2:
- Two registration nights will be held for all teams that have already submitted their team’s application along with payment. During this time, teams should bring the team roster and waivers and proof of age for all players.
- Dates: TBA
- Registration will be held inside the Discovery Sports Center
- Paperwork may be submitted prior to these dates if you have collected the appropriate information from your team
